Job Title: Care Co-ordinator

Reports to: Registered Manager

  1. Answer the telephone and assist with day-to-day problems as required, liaising with care and support, care supervisors, service users, relatives and social workers etc.
  2. Take details of requests to provide a service and liaise other ACASA staff, other Care at Home Providers and subcontractors as required to ensure that all care packages from HCC are taken on.
  3. Ensure that all Care enquiries are responded to within two working hours and no care packages from HCC are declined.
  4. Contact the area Care Supervisor with details of new care packages in your area within 1 hour of referral/acceptance so they can make contact with new service users on the same day that a referral is received and arrange a meeting with them and their representatives, if applicable, within 48 hours
  5. Enter new service users details onto the computer system.
  6. Plan and maintain the care rota for your area, ensuring that all care calls are covered
  7. Monitor the carers’ rotas, ensuring that all shifts are full, as far as practically possible.
  8. Liaise with the HR and Recruitment Team and your Team Care Supervisor to ensure the smooth integration of new Care and Support workers into your care team.
  9. Liaise with other Care at Home Providers and Subcontractors, if applicable to ensure that no HCC care package is declined.
  10. Encourage/remind Care and Support Workers to login and out of the CM2000 electronic monitoring system, where required to ensure Aura targets are met.
  11. Liaison to with Adult Services, the REACT team, Health Centres, OT services, local third sector services the local community to promote optimum service user outcomes.
  12. Generate scheduled and ad hoc reports as required
  13. Provide help and support to other care co-coordinators as required.
  14. Provide holiday and sickness cover to other care co-coordinators, as required
  15. Provide weekend cover in the office and out of hours cover as per the weekend and out of hours’ roster.
  16. To support the Manager in the recruitment of new staff (conducting interviews and telephone interviews)
  17. To support the manager in the procurement or procurement requests for additional resources
Person Specification

Computer literacy
Flexible approach
Excellent verbal and written communication skills
Team player
Ability to work in a busy pressurised environment

Background in care
Hold an NVQ or Health and Social Care Diploma in Care at level 2 or 3

Key Performance indicators

All Care calls within your area are covered at all times

All Care and Support Workers shifts are as full as practically possible

All care packages referred from HCC are taken on

All care enquiries are responded to within two hours

Details of new care packages are passed to the team Care Supervisor within one hour of the referral
