Care Supervisor

Job Title: Care Supervisor

Reports to: Registered Manager

Duties:
  1. Make contact with new service users in your area on the same day that a referral is received and arrange a meeting with them and their representatives if applicable, within 48 hours. At this meeting, you should put together the following:
    • An independence plan, based on the Personal assessment and Personal plan, put together by social work teams,
    • A risk assessment, based on the outcome plan received
    • A short term plan to meet any goals not yet achieved or goals that are realistic to achieve over the next 12 weeks.
  2. Carry out Continual assessments of Individual’s needs at predefined times i.e. 12 weeks/26 weeks, at predefined times i.e. 12/26 weeks as and when required due to changes in service user needs.
  3. Liaison to with Adult Services, Health Centres, OT services, the REACT teams local third sector services and the local community to promote optimum service user outcomes.
  4. Liaise with the HR and Recruitment Team and your Team Care co-ordinator to ensure the smooth integration of new Care and Support workers into your care team and assist with their induction into the company.
  5. Carry out two monthly supervision meetings and annual Appraisals with the Care and Support workers in your team.
  6. Carry out team meetings with your Care and Support workers every three months
  7. Liaise with the HR and Recruitment Team and your Manager to deal with employee issues or concerns appropriately and as per company policy
  8. Encourage/remind Care and Support Workers to login and out of the CM2000 electronic monitoring system, where required to ensure Aura targets are met.
  9. Provide emergency cover for Care and Support Worker shifts, as required
  10. To support the Manager in the recruitment of new staff (conducting interviews and telephone interviews)
  11. To support the manager in the procurement or procurement requests for additional resources
Person Specification

Essential
Significant Care Experience
Hold an NVQ or Health and Social Care Diploma in Care at level 3 or be prepared to work towards one Computer literacy
Flexible approach
Excellent verbal and written communication skills
Team player
Ability to work in a busy pressurised environment

Desirable
Previous supervision experience
Domiciliary Care experience
Hold an NVQ or Health and Social Care Diploma in Care at level 2

Key Performance Indicators

All Service Users in your area have the following:
  • An independence plan, based on the Personal assessment and Personal plan, put together by social work teams,
  • A risk assessment, based on the outcome plan received
  • A short term plan to meet any goals not yet achieved or goals that are realistic to achieve over the next 12 weeks
Continual assessments of Individual’s needs are carried out at predefined times i.e. 12 weeks/26 weeks and as and when required due to changes in service user needs.

Two monthly supervision meetings and annual Appraisals are carried out with the Care and Support workers in your team.

Team meetings with your Care and Support workers are held every three months

CM 2000 Aura targets are met.
